RMA Authorization Form                                         

 

1. Must complete the RMA form with detailed problem of the products provided.

2. All request serial numbers must match with the original invoice.

3. RMA number is valid for 7 days from the date of issue.

4. It is the customer’s responsibility to call if NO RMA# or response was received after 24 hours.

 

RMA TIPS: To speed RMA process, please describe the problem as detail as possible. Please attach another sheet if need.
